Transitional Care of Medically Complex Children Program (TCMCC Program)
In November 2022, Safehaven developed the Transitional Care of Medically Complex Children Program (TCMCC) in partnership with Holland Bloorview Kids Rehabilitation Hospital.
The program provides safe, community-based care for children transitioning from lengthy hospital stays to home and community.
By moving children out of the hospital when they can be safely cared for in the community, we are alleviating pressures from “alternate level of care” patients (those in hospital only because more appropriate care can’t be found elsewhere).
